In order for your child to accompany his or her class on field trips organized by the school, you are required to sign the release section of this form. Please note that we will be unable to take your child on any school trip unless this section is completed.
I/We give permission for my/our child to accompany his/her class on field trips arranged by Fern Hill School during the academic year.
Municipal Freedom of Information and Protection of Privacy Legislation requires that you complete the permission section of this form dealing with the use of class/individual photographs, videos, and student work.
Each year we publish a school directory with each student's name, parent's name(s), address, home telephone number and e-mail address. Please indicate whether or not you wish information on your family to be published.
I/We give permission for my/our child's photographs, videos, and/or work to be displayed in the school, included in the yearbook, and used as part of a school related activity.
I/We give permission for my/our child's photographs, videos, and/or work to be published in the school, included in the school newsletter or community and local papers/media.

At minimum we would need the following for the School Directory: child's name / class / at least one parent's name / a way to contact parent(s) (email or phone number)
Upon receipt of an "Offer of Admission", parents will be responsible for providing required signatures on all necessary registration documents. Registration will not be considered finalized, and your child's spot will not be guaranteed, until all required signatures and supporting documents are received by the office.
